Local Rules of the First and Fourteenth Courts of Appeals require the following statement be attached to the notice of appeal filed with the trial-court clerk. A copy of the notice of appeal and this statement must be filed with the Clerk of the First Court pursuant to Texas Rule of Appellate Procedure 25.1(e).
Once the trial-court clerk has assigned the appeal to the First Court, the appellant must file a docketing statement with the Clerk.
If the Court refers a civil appeal to mediation, three forms must be filed with the Clerk at appropriate stages in the mediation.
Once the trial-court clerk has assigned the appeal to the First Court, the trial-court clerk and court reporter must file an information sheet with the Clerk.
