Local Rules of the First and Fourteenth Courts of Appeals require the following statement be attached to the notice of appeal filed with the trial-court clerk. A copy of the notice of appeal and this statement must be filed with the Clerk of the Fourteenth Court pursuant to Texas Rule of Appellate Procedure 25.1(e).

Once the trial–court clerk has assigned the appeal to the Fourteenth Court, the appellant must file a docketing statement with the Clerk.

Once the trial–court clerk has assigned the appeal to the Fourteenth Court, the trial-court clerk and court reporter must file an information sheet with the Clerk.
